  1. You have the right to receive considerate and respectful care at the Rutgers Camden Student Wellness Center.
  1. You have the right to an explanation of your diagnosis, treatment and prognosis in terms you can understand.
  1. You have the right to receive the necessary information to participate in decisions about your care and to give your informed consent before any diagnostic or therapeutic procedure will be performed.
  1. You have the right to refuse treatment, except as prohibited by law, and to be informed of the consequences of making this decision.
  1. You have the right to expect that your personal privacy will be respected by all staff at the Rutgers Camden Student Wellness Center.
  1. You have the right to expect that your medical records at the Wellness Center are strictly confidential.  Information will be released only with your written consent to anyone not connected with your care, or in cases of medical emergencies, certain state-mandated reporting of infectious disease or in compliance with a legal subpoena
  1. You have the right to know the names and positions of people involved in your care by official name tags or personal introductions.
  1. You have the right to a full explanation of any research or experimental procedure proposed for treatment and the opportunity to give your informed consent before any procedure will begin.
  1. You have the right to ask and receive an explanation of any charges that may be made by the Wellness Center, even though they may be covered by insurance.
  1. You have the right to obtain another medical opinion prior to any procedure.

  2. You have the right to review any medical records created and maintained by the Rutgers Camden Student Wellness Center regarding your care and treatment.
  1. You have the right to request that health care service be communicated in your native language if you do not speak English.

  2. You have the right to change providers.

  3. You have the right to discuss advanced directives (living will) with your provider.
  4. You have the right for all persons in the Rutgers Camden Student Wellness Center to wear appropriate PPE to protect you from possible exposure to COVID 19.

PATIENT RESPONSIBILITIES

  1. You are responsible for providing accurate information about your past medical history.
  1. You are responsible for asking questions if you do not understand the explanation of your diagnosis, treatment, prognosis, or any instructions.
  1. You are responsible for providing information necessary to process your insurance claims.

  2. You are responsible for any charges billed to you.

  3. You are responsible for following all rules and regulations that are posted within the Wellness Center.

  4. You are responsible to behave respectfully to all staff, providers and all others present in the Wellness Center.

  5. You are responsible for providing accurate information regarding exposure to COVID 19 and being or having been positive for COVID 19.
