The Wellness Center offers psychological assessment for students. Testing is primarily utilized for psycho-educational purposes to help students in deepening their understanding of how they learn, think, feel, behave, and relate in the world.
Testing is also used to assist students with clarifying their diagnosis and answering outstanding questions related to their past or current level of functioning.
Testing Services evaluate the following issues:
- Attention Deficit/Hyperactivity Disorder (ADHD)
- Emotional Support Animal (ESA) Evaluations
- Learning Disabilities
- Medical Disabilities (in consultation with medical staff)
- Psychological Disabilities
- Traumatic Brain Injury

For Accommodations
For individuals who want to complete an assessment and who are seeking educational accommodations from the University you should:
- Contact the Office of Disability Services (link) to complete a Pre-Screen Assessment. The pre-screen will generate a Service Provider Referral Form;
- Once the above process is completed, confirm the Pre-Screen Assessment has been sent to the Wellness Center. Call (856) 225-6005 to schedule a consultation session with the Assessment Coordinator in the Student Wellness Center or schedule an online appointment.
- You will then be assigned to an evaluating clinician to complete the appropriate assessment process.

Service Provider Referral Form:
This form is completed by the Office of Disability Services to document the reason for your referral for testing services, provides some preliminary background information to assist in the test battery development process and documents your response to formal and informal interventions. The packet also includes an authorization form that allows the Office of Disability Services and the Student Wellness Center to share information with each other.
